Title: Addressing Medical Errors through Morbidity Mortality Improvement Rounds and Root Cause Analysis: A Team Approach

Description:

Medical errors are an important cause of negative outcomes for patients and clients as well as members of the veterinary healthcare team; however, reporting and addressing errors to avoid a "blame and shame" culture can be challenging. This limited enrollment session will provide a systematic approach to address major medical errors with your hospital team by using a Root Cause Analysis approach. This approach helps the healthcare team identify the systems errors that contribute to a medical error occurring, rather than focusing on the individual making an error. When systems errors are identified, the team can work together to minimize or eliminate the chance of repeat error, and develop a healthier hospital culture in which team members are supported.
